MEDICAL FORM
Attention Parents: All children being registered for camp MUST COMPLETE a medical form and return it to the Camp Director. Medical Form (Download PDF) print and mail.

In accordance with New York State regulations, parents and guardians are required to review the official 3601 Regulated Children’s Camps in New York State (11/25) publication issued by the New York State Department of Health. This document outlines the rights and responsibilities of campers and camp operators as required under Section 7-2.5(p) of the State Sanitary Code.
